COVID-19 and the CARES Act

Your Plan adopted important provisions of the CARES Act that may allow you to make an emergency withdrawal or have your loan repayments due in 2020 delayed.

To be eligible for either the Coronavirus Related Distribution (CRD) or delaying loan repayments that are due in 2020, you must be able to self-certify under one of the following:

  • I have been diagnosed with the virus SARS-CoV-2 or with coronavirus disease 2019 (COVID-19) by a test approved by the Centers for Disease Control and Prevention; or
  • I have a spouse or dependents diagnosed with such virus or disease by such a test; or
  • I have experienced adverse financial consequences stemming from such virus or disease as a result of:
    • Being quarantined, furloughed, or laid off
    • Having reduced work hours
    • Being unable to work due to lack of child care
    • The closing or reduction of hours of a business I own or operate

Coronavirus Related Distribution (CRD) Form – use this form to request up to a $100,000 distribution

Coronavirus Related Loan Repayment Delay Request – use this form to request to delay repayments that are due in 2020. If using this Form in conjunction with taking a new loan, the loan initiation fee will automatically be waived.

Coronavirus Related Loan Initiation Fee Waiver Request – use this form to request the loan initiation fee be waived if you are taking a new loan and not going to delay repayments

Incoming Assets Form (PDF)

Use this form to:

  • Roll money over from other retirement savings accounts into your Plan
  • Decide which investments you want the rollover money to go into

Payout Request Form (PDF)

Did you know you may be able to apply for a distribution on-line? Just log in to your account and select “Manage account,” then “ Withdraw money.”

Use this form to:

  • Select your payout options when you end your employment or retire
  • Request a payout
  • Stop current payouts
  • Change a current payout
  • Restart a payout
  • Required Minimum Distribution (RMD) required at age 70½

Beneficiary Change Form (PDF)

Use this form to change beneficiary information:

  • Select primary and contingent beneficiaries
  • Change how your money will be divided between your beneficiaries
  • Remove and add beneficiaries

Automatic Deposit Authorization (PDF)

Use this form to:

  • Add or change your checking or savings information for automatic deposits to your bank account

Withholding Certificate for Pension or Annuity Payments (IRS Form W-4P) (PDF)

Use this form to:

  • Determine how much Federal income tax you would like to have withheld from your pension or annuity payments

Beneficiary Distribution Form (PDF)

Use this form for the initial claim, if you're a beneficiary, to report the death of a Plan member. Please remember to submit an original or certified copy of the death certificate and a copy of your driver's license.
